We know the process of obtaining medical cannabis can be confusing. Registering with a cannabis provider varies depending on your situation. Whatever your circumstance, we try to keep it as simple as possible.


Step 1
Patient Application

Every new patient to Shelter Market requires a Patient Application, which can be downloaded below. This form is required to be filled out by either the patient or a caregiver. Please note that ALL mandatory sections be completed to avoid delay or rejection.

Step 2
Medical Document

All patients require a medical document. Please follow the link that applies to you:

New Patients

Medical documents can only be completed by a health care provider.

If submitting by fax, medical documents must be secure-faxed directly from your health care provider's number. Alternative numbers will not be accepted. If submitting by mail, only original medical documents can be accepted.

Transferring Patients

To transfer your medical document from another provider to Shelter Market, you will need to either request your current provider to return your original medical document and provide it to us yourself; or request your current provider to transfer your original medical document directly to us.

Splitting Authorization

To access cannabis from more than one provider, your original medical document must be returned from your current provider, and two (2) amended medical documents must be obtained from your health care provider.

ACMPR Licences (interim supply only)

At this time, we are not authorized to provide seeds, clones or other starting materials to ACMPR authorized patients. You will need to submit a copy of your Registration Certificate, and a statement to the effect that a) the applicant ordinarily resides in Canada, b) the information in the Application and the Registration Certificate is correct and complete, and c) the application is being made for the purpose of obtaining fresh or dried cannabis or cannabis oil only for the applicants personal use, and d) the Registration Certificate is not being used to seek or obtain those substances from another source.

What happens after my documents are submitted?

We’ll contact you as soon as we receive your completed forms to let you know they arrived safely. Next, we’ll contact your health care provider to verify your medical document. Once everything is confirmed, we’ll send you a notification of registration and you can begin placing orders immediately.
